Various forms are required for different troop activities. Please ensure all forms are completed accurately and submitted on time.
Forms for Massawepie Summer Camp
- Medical Form - Required for all Massawepie participants
- Permission Slip - Required for all Massawepie participants
- Swimmer Classification - Required for all Massawepie participants
High Adventure Forms
- High Adventure Medical Form - Required for all High Adventure Camping
- Physical Fitness Verification
Sea Base Specific Forms
- Diving Certification - Required for Sea Base diving itinerary adventures
- Swimming Qualification
General Troop Forms
- Annual Health and Medical Record - Required for all scouts for general participation in troop
- Parental Permission Form - For trips and activities
Trip Activity Forms
- Tour Permit - For Leadership to fill out for all trips outside BSA camps
Important: All forms must be completed and submitted before scouts can participate in the respective activities. Medical forms must be signed by a physician and should be updated annually.
How to Submit Forms
Completed forms can be:
- Submitted to the Scoutmaster at a troop meeting
- Emailed to [email protected]
- Given to the appropriate activity coordinator
Please keep copies of all submitted forms for your records.
